Your Personal Anti-Aging Program and Fees

• Upon scheduling your evaluation we will collect a $ 600 credit card deposit by telephone. This deposit will cover your initial comprehensive evaluation and follow up evaluation to discuss your lab results.

• We require at least 48 hour cancellation rescheduling. .

• During the first visit, will conduct a complete medical history and obtain physical measurements. This information will allow us to asses your current status and to make the appropriate laboratory request for your particular needs. Lab work will be obtained.

• During the second visit, we will discuss your laboratory reports and make the appropriate recommendation with regard to therapy based on a thorough discussion. The doctor will answer any questions that you have regarding your program.

• Our goal is for you to have a complete understanding of all the scientific information we have collected and your options so your decision to proceed will be a fully informed one.

• If you elect to start any type of hormone replacement program there will be an additional Professional fee of $ 1400 to cover all of your follow up visits, phone calls and any other encounters during the dose titration phase which normally lasts 3 to 4 months.

Follow Up Evaluations

• Once therapy is optimized, maintenance visits will be approximately at 3 months intervals.

• At these quarterly visits, a targeted panel of laboratory tests and physical measurements will be obtained in order to monitor therapy and track your progress. The costs of the laboratory testing will be billed to you by the laboratory.
